Application for DOELAP Radiobioassay Vendor Qualification

Welcome to the DOE Laboratory Accreditation Program (DOELAP) application website for Direct (In Vivo) and Indirect (In Vitro) radiobioassay programs. The link, user ID, and password for this website is assigned to your facility in an email from the radiobioassay Senior Technical Manager (STM) to a facility point of contact (POC). Access to the application process is dependent on the information supplied to DOELAP.
 
After the primary contact or a person responsible for completing the online application is finished, they will forward the application to the STM for a brief initial review. If the application appears to be in order, the STM will forward it to the radiobioassay program's authorized representative. If satisfied, the authorized representative approves and forwards the application back to the STM to finalize the application. This submission is equivalent to a signed hardcopy application. At each step, the application is automatically emailed to the appropriate POC based on information obtained from the application.
